New Life Smile Privacy Policy

For each visitor to our Web page, our Web server automatically recognizes information transmitted from the visitor's web browser (where possible).

We collect the e-mail addresses of those who communicate with us via e-mail, aggregate information on what pages consumers access or visit, user-specific information on what pages consumers access or visit, information volunteered by the consumer, such as survey information and/or site registrations, name and address, telephone number.

The information we collect is used to improve the content of our Web page, used to notify consumers about updates to our Web site, used by us and our partners to contact consumers for marketing purposes, disclosed when legally required to do so, at the request of governmental authorities conducting an investigation, to verify or enforce compliance with the policies governing our Website and applicable laws or to protect against misuse or unauthorized use of our Website.

If you supply us with your postal address on-line, you will only receive information and materials for which you provided us your address.

Persons who supply us with their telephone numbers on-line will receive telephone contact from us or our approved partners.

I. Information Collection Purpose

We collect your contact information including email address and phone number in case we need to contact you about the information you submit to us. This will ensure we are able to deliver to you any requested material or free offers. It also allows us in some cases to contact you to confirm you are qualified to receive our free offers and that the materials you have requested are best suited to your specific situation. Along with any resource information, we may also suggest other options that may be available to you or a loved one.

II. Eligibility Requirements

Must be adult (18+) women and men with at least 6 teeth on the desired arch (upper or lower) whose lives will be changed by receiving a free smile makeover.

The applicant must agree that this program does not replace or fix any dental issues.

By filling out an application, an applicant is not guaranteed to be chosen for a free smile

Our New Life Smile team reviews all applications for minimum requirements. If initially qualified, the applicant enters a pool from which our selection committee awards free smile makeovers. The applicants are then contacted by a smile specialist who will walk them through the process of receiving their free smile makeover.

By submitting an application, the entrant authorizes the use of his/her likeness and/or story for any and all purposes.

Applicant acknowledges that New Life Smile Inc. or its partners will not be held liable for any loss, damages or injury associated with applying for, accepting, or using their new smile.

Applicant acknowledges that any dispute as to the fit, shape, color, or any other feature of the free smile makeover may not be corrected, and the applicant's satisfaction is not guaranteed.

Applicant acknowledges that once New Life Smile Inc. or its partners have marked the case as closed, no further work will be done in the applicant's case without payment.

Applicant acknowledges that they can be disqualified from the program at any time if they: do not respond to requests from New Life Smile Inc. and its partners within 30 days, do not maintain a professional demeanor with the staff at New Life Smile Inc. or its partners, attempt to use any method to disparage or defame: New Life Smile Inc., its partners, or their staff.

Applicant acknowledges that they can be disqualified from the program at any time if they do not notify New Life Smile Inc. and its partners of any changes to contact information such as phone number, email address, or mailing address.

Applicant acknowledges that New Life Smile Inc. or its partners retain the right to make any and all decisions regarding the services being rendered and/or to cancel any agreement or work being done for any reason whatsoever.

Applicant acknowledges the application process takes up to 90 days. Applicant agrees not to contact New Life Smile Inc. or its partners to check on the status of their application. Applicant also agrees not to submit another application for our smile giveaway for at least 90 days.

Photo Submission Requirements

You understand and agree that you are required to submit usable photos of your teeth (from nose to chin, high resolution, well-lit, in focus, displaying all ‘smile teeth’ and a full gum-line) in order for us to consider you for a no cost smile makeover. Without these photos, your submission is not considered complete. We reserve the right to discard any application where photos have not been submitted. You may be required to submit new photos, if, in our sole discretion, they are deemed to be unusable for any reason.

II. Information Collection Method

The personal information we collect is limited to that which you submit to us via our web contact forms, chat sessions, or if you contact us by phone.

With respect to Ad Servers: Third party vendors show our ads on sites on the internet. Third party vendors use cookies and may serve ads based on a user's prior visits to our website. Users may opt out of Google's use of cookies by visiting the Google advertising opt-out page. Or they may opt out of a third party vendor's use of cookies by visiting the Network Advertising Initiative opt-out page.

Upon request, we offer visitors the ability to have inaccuracies corrected in contact information, communications that the consumer/visitor has directed to the site.
Consumers can have this information corrected or opt-out of future communications by sending an email through our Contact form.

With respect to security: When we transfer and receive certain types of sensitive information such as financial or health information, we redirect visitors to a secure server and will notify visitors through a pop-up screen on our site.
